Efficacy of Canine-Assisted Therapy in Children with Autism Spectrum Disorder (ASD): A Systematic Review and Meta-analysis of Randomized Clinical Trials

Abstract

Introduction: Autism Spectrum Disorder (ASD) is characterized by persistent deficits in social communication and interaction. Dog-Assisted Therapy (CAT) emerges as a complementary intervention with the potential to improve therapeutic engagement and outcomes in ASD. Objective: To evaluate the efficacy of CAT in children with ASD, compared to control groups, focusing on executive functions, social skills, and emotional regulation. Methodology: Systematic review following PRISMA 2020. Searching PubMed, Scopus, Web of Science, and PsycINFO (2015-2025) for randomized controlled trials (RCTs) comparing CAT versus controls in ASD. Meta-analysis with a random-effects model. Results: Two RCTs (n=82 participants) met the criteria. The meta-analysis demonstrated a significant effect of ACT (SMD=0.78; 95% CI: 0.13-1.43; p=0.019), with moderate heterogeneity (I²=43.8%). The study by Hill et al. (2020) showed improvement in task behavior, while Van Der Steen et al. (2024) reported significant improvements in emotional regulation. Conclusion: ACT shows promising benefits for children with ASD, but current evidence is limited by the small number of studies. More RCTs are needed to confirm these findings.
