Abstract
This abstract highlights the process of creating an instrument developed in the context of a university extension. The initiative involved academics from the Liga Acadêmica de Cuidados em Feridas (LACEF) in partnership with professionals from the Ambulatório de Feridas e Pé Diabético de Arapiraca (A.F.P.D.A.), with the objective of developing an essential tool to ensure a safe transition from outpatient care to home care. As a result, ten discharge care plans were developed, considering the specificities of each etiology. This approach aims to reduce the chances of relapse of users after discharge, in addition to stimulating and strengthening their ability to perform self-care.

Electrotechnical Technician from the Federal Institute of Alagoas - Palmeira dos Índios Campus, since 2022. She is currently a student of the Bachelor's Degree in Nursing at the Federal University of Alagoas - Campus Arapiraca Sede. She participates as a collaborating liaison in the Academic League of Wound Care (LACEF) of the Federal University of Alagoas (UFAL). She worked as a scholarship student in the extension project offered by UFAL, in partnership with the Municipal Health Department of Arapiraca/AL, Education through Work Program PET-Health: Management and Assistance in Tutorial Group 01: Health Education Management. As a member of WG 01, she participated in the development of two technological products to be published on the eduCapes platform with the purpose of constituting two Educational Products, namely a WebQuest called "WebQuest Health Education" and an explanatory video entitled "Educational processes in the context of SUS health practices". Currently, she is part of the Work, Social Being and Nursing Study Group, acts as a member of the Academic Center The Voice of Nursing and is a collaborating student of the extension project Discharge Care Plan for Patients with Injuries at the Wound and Diabetic Foot Outpatient Clinic of Arapiraca. She participates as a scholarship student in the edition of PET-Saúde: Equity through the project "All for them, nothing without them: building equity with SUS workers"
