Medicamentos potencialmente inapropriados para idosos internados em unidade de terapia intensiva

Abstract

Objective: The objective of the study was to identify the factors associated with the use of potentially inappropriate medications for elderly patients admitted to Intensive Care Units (ICU). Methods: Retrospective cross-sectional study, carried out in a state public hospital from July to December 2019. Potentially Inappropriate Medications (PIM) for the elderly were identified in prescriptions by consulting the 2019 Beers Criteria and categorized into: use of 1 or ≥2 PIM. Results: The prescriptions of 189 patients were analyzed, with a median age of 74 years and an interquartile range of 17, 50.3% of which were female. The length of hospital stay had a median of 8 days and an interquartile range of 12. Among the 189 prescriptions, 86.2% contained PIM, 58.3% contained at least 1 PIM and 41.7% 2 or more. The most prescribed drugs were those that act on the alimentary tract and metabolism: Metoclopramide (72.5%) and Omeprazole (29.1%). An association was found between prescriptions containing 2 or more PIMs with diabetes mellitus (DM) (PR = 1.61; 95%CI: 1.11 - 2.33). Conclusion: The study showed a high frequency in the use of PIM for the elderly admitted to the ICU, and patients with DM as a comorbidity had 2 or more PIM in their prescriptions.
